Cafeteria Registration & Information Form
Healthy Kids Act/Physical Activity Contract (complete for ALL students)
6th & 7th Grade Acceptable Use Policy Acknowledgement
8th Grade (1:1 iPad) Acceptable Use Policy Acknowledgement
Forms that need to be printed, completed and turned in to the Main Office by August 12. (The Regis Main Office will have printed copies of these forms available.)
Fees Form (print and submit with payment for ALL STUDENTS)
Free & Reduced Meals Forms (optional — print and submit)
Food Allergy & Anaphylaxis Care Plan Form (if needed for your student)
Student Physical Activity Examination Form (7th & 8th grade athletes, print and submit)
Concussion Form – ALL 7th & 8th grade athletes MUST READ and SIGN this form (in addition to the parent signature) before participating in school athletics. Form must be printed, signed & submitted.
Asthma Action Plan (for students with asthma – print and submit)
Grade Level Forms
YAHOOO Form (required for 6th grade students)
YAHOOO Parent Volunteer Form & Background Check (for 6th grade parents looking to volunteer at camp)
Proof of Tdap and Meningococcal Vaccine (required for 7th grade students — the School Nurse has contacted those families who are still in need of providing the vaccination records to the school)
